There has been some information in the media recently about data sharing in the NHS. If you would like to opt out of your data being shared for research and planning purposes, the easiest way to do this is via the NHS App.
• Select the "Your Health" tab at the bottom,
• Scroll down to "Choose if data from your health records is shared for research and planning’"
• Click on "Start Now"
You do not need to take any further action if you opt out using the above method.
Alternatively, use the following link to download and complete the form, this must be returned to us BEFORE 23rd June. NHS Digital opt out form

We would suggest calling after Tuesday when the phone lines are typically quieter. You could also complete an econsult form or email us at NGCCG.A85016@NHS.NET.
